How do patients come to St. Joseph's Hospice?

In many different ways, but always by the choice of the patient or their family. 

Their families are advised by hospital discharge teams, social workers and specialist palliative care nurses to consider St Joseph's Hospice. Others are at home (or in a care home) but are reaching a point where they cannot stay there. They may be advised by a G.P, district nurse or other community health practitioners to consider our Hospice, or they and their families may already know of us.

We welcome visits from families before making a decision, and in some cases our Director of Clinical Services may visit prospective patients in hospital or their own homes. In almost all cases an admission requires the prior consent of the Clinical Commissioning Group (CCG). Our Director of Clinical Services is always happy to help patients and their families negotiate this complex system.
